I guessed that he was either using thicker washers like we see with the OEM mount, or maybe he was using washers to space the shock down some more like James, Donny, myself and a couple others have done.
I've broken the shock internally. I've also significantly bent the threaded portion at the base of the nut after the bushing failed and the rod and lower washer under the bushing hit the side and inside of the lift spacer. It would seem that you would have seen a different failure if it was going to brake "normally".

The more we talk, and the more I think about it, the more I think the problem is specific to that particular shock.
